ASSUMPTION OF RISK FORM
I am aware that participating in any of the Coffs Coast Adventure Centre’s activities can be dangerous in nature, involving possible RISKS OF INJURY. I am also aware that there is a risk of an adverse effect to a current or past injury and/or medical condition. Activities I may participate in are as follows:
ONSITE ACTIVITIES
Outdoor Games
Camp Fire
The Coffs Coast Adventure Centre takes all reasonable steps to provide safe and enjoyable facilities, games and activities. However, the Coffs Coast Adventure Centre will accept no liability or responsibility for any loss or damage to property, or, injury of or to any person save for any loss or damage directly caused by the negligent act or omission of the Coffs Coast Adventure Centre. The terms hereof shall serve as a release and assumption of risk for my heirs, estate, executor, administrator, assignees, and for all members of my family.
I recognize there are rules and regulations, which govern The Coffs Coast Adventure Centre’s facilities, games and activities, and these rules and regulations are in place for my safety, and the safety of others. I agree:

• To abide by these rules and regulations, for my safety and for the safety of those around me.
